Hey Rj, just make sure the I/We box has the name the same as your holding and the correct address of the holding. They will pick up the number of sharex from the registry.
Leave Name of Proxy blank and mark the Chair box. Also mark the Important for Resolutions 4 and 5 box. Down the bottom put the date, sign and print your contact name and phone. Good Luck....
