Usually fishing occurs when there is a twist off in the pipe. If there is a flaw in the pipe or if they are drilling haphazardly they pipe gets a crack in it, and as it turns during drilling the crack gets bigger until it "twists off" leaving 2 separate bits of pipe in the hole.
I don't believe they try to fish for small debris, since they can usually flush it out by using extra drilling mud.
