I've just set up a US$ account at the Commonwealth Bank - amazingly you can have an account created instantly online via a Netbank login. No account keeping fees. A$30 to deposit a cheque but they recommend you make an over the counter (teller) deposit. No transaction fee for a transfer to a linked account - only conversion fee if to an Oz$ account.
