Contacted Moly............very helpful......gave me the boardroom HOTLINE for QMN offer 1300105041.....very helpful.....offered to immediately resend the personalised form which had to be signed and returned by 19/1.
However because of the PARLOUS state of Australia Post you can now contact your broker in my case COMMSEC and they can accept on your behalf just need to send a confirmation email to them and the cheque will arrive in the mail in about 3 weeks.
