If you're filling out this form electronically, and assuming you are using Microsoft Word, you can easily replace an empty tick box with a checked box by selecting the "Insert" tab, and "Symbol", select appropriate symbol and insert. (Of course you can adjust font size, if you think the default is too small etc).
