My take is Stevenson was referring to undercall of ore being delivered to the plant , ie the ore was not as high a grade as indicated in the resource resulting in a lower than expected reconciliation in the plant.
Therefore while the plant's throughput (tpa) of ore was adequate, the lower than expected grade of ore entering the plant would mean a lower than expected amount of recovered product.
My recollection is the ore at Mt Carbine was in pipes surrounded by country rock I imagine that would be a difficult scenario to mine without causing either dilution if country rock was mixed with ore or lower that expected ore production rates if time was taken to support the stope walls was to occur.
Just my take and not a mining professional.
