Not to make light of these breaches, but this cowboy attitude is pretty rife amongst juniors and even some of the majors. The regulator is being kept pretty busy.

I dont see a long term problem but this will cause a delay and perhaps incurr extra costs. This will be a wake up call for them to behave more professionally in future. Again, lazy, poor quality management. They are being paid well but give sub standard performance.
