mate, the SPP form is individual as it has BPAY reference for each holder so you will need the specific one for yourself.
Have you tried ringing the details below ?

They will ask for your personal details including HIN and they will e-mail a copy to you. From there, you can make BPAY straightaway with your specific reference number.
