    I stand corrected on this comment as there's two proxy forms in the mailout, the pre-filled one as a loose sheet at the front of the documents and another blank one embedded inside the larger document right at the back.  I still think it's misleading though to include this as a pre-filled document but have no idea if this is common practice or not.
