After a quick look at the documents above the mine changed their scrubbers which caused the increase in waste products being discharged to the water but prior to that they were within their limits (wouldn’t like to be the process engineer that signed that off)

Then there was a leak that took 3 days to find. Poorly maintained earth bund walls (it’s a bit hard to see the bunds from some of the pictures but the ones I can see look like earth bunds)or poorly lined bunds can cause this very easily. 

They also mentioned a large rain event didn’t help which could have contributed to the bund wall issue as well as reducing on site retention (lack of bund size due to leak) so there was no treatment for that water that leaked. 

the key takeaways from this (for me) is keep up maintenance and don’t make changes to the plant unless you are sure if the repercussions.
