I don't use a broker and do my own research so all I have is the paper form that Melbana sent me.
The only option with the form is a cheque - no problem for me since I still have a cheque account.
What is that say the 25 year olds
So for those in a similar position I suggest you organise it soon because the form and the cheque need
to be in the hands of the company by 31 August
