I think they are very serious about raising this money and suspect the reason it is so hard to find the application form is that they are probably already oversubscribed. They are raising only $5M so it is likely they will achieve that from their "appropriate" shareholder base as suggested by Clause 2.9 in the prospectus. I do not think they need any subscriptions from Joe Public at this stage

"The Directors will allocate Shares under the Public Offer at their sole discretion with a view to ensuring an appropriate Shareholder base for the Company going forward."

The best you can hope for is for some share price weakness on relisting (and that you can pick some up close to the 15c cap raise price - if you think that is good value).
