nope, that't not hard to imagine if you have studied chain of command, checking etc. - it's common and it's a BIG problem.
If you are the 2nd person in a check queue out of say 3 and you spot an issue, even if you aren't entirely sure - you are likely to raise the issue.
If you are 48th in the line of checkers out of 50 - it's highly unlikely that you will raise an issue even if you are almost certain.
I can't remember the details, but, I studied it in the military and in medicine - I have see it more than once in hospital care (or non care) and, I have read many failures in military history from it.
NASA from memory has done some work on it.
That part of it is not surprising.
