Looks like both the CEO and Chairman have been marched in to an Accountability Meeting with the Chief Inspector of Mines in Brisbane.

Undermining the Site Senior Executive and the mines health and safety systems
Taking on responsibilties of the Site Senior Executive
Even more concerning, seems the company then got caught out misleading the mines department and trying to cover it up. MRE's have to be communicated to all staff and contractors so the entire site is now being forced to sign off acknowledging how bad these guys suck, lol.
Now banned from any direct communications with any staff or contractors.
