    re: options offer....6 for 1 Ok ive downloaded the Disclosure document, for the options offer it says to fill out and Complete the Entitlement and Acceptance Form which is attached, but i cant find it!!! help!! all i can find is the forms for the Public Offer... but nothing for the options offer, has anyone else found it , on what page? or has it been left out by mistake?
    Can anyone help?
